This form must me accompanyied by a signed consent form from the power of attorney or legal wife of the decease. TIME SENSITIVE TESTING. Contact the the funeral home and inform them of the impending DNA Collection. We will call and arrange collection time after the test is ordered.  A non-refundable 25% fee is collected if the collector attempts but, due the condition of the body, collection is deemed futile. Overage will be promptly refunded.
